- Names of the bride and groom

The request must include: name, address and other identifying data of the individual, a description of the request, preferred form of communication, actions under Articles 15–22 of Regulation (EU) 679/2016, signature, date of submission of the notice, and correspondence address. If the notice is submitted by an attorney/representative, an explicit power of attorney must also be attached.
